Role: Case Manager (Wellness Duties & Responsibilities)

Reports to: Program Manager, Seni VanHook

Location: Access Center 503 Olympic Blvd., Santa Monica, CA 90401

Schedule: Monday - Friday, 8:30 am - 5 pm (1st and 2nd Wednesday of each month 8:00 am to 4:30pm)

Status: Full-Time, Non-Management, Non-Exempt

Summary

Using evidence-based practices such as harm reduction, housing first, and trauma-informed care, the Wellness Case Manager supports Clients' housing stability, self-sufficiency, and community integration. The Wellness Case Manager partners with Clients who are currently homeless to assess and address barriers to housing placement. The Wellness Case manager implements groups and collaborates with clients to learn independent living skills and other life skills vital to successfully transitioning from homelessness into permanent housing.

Essential Duties and Responsibilities:

Under the supervision of the Program Manager, the Case Manger is responsible for:

1. Develop effective, trusting relationships with clients using a client-centered approach using best practices that include motivational interviewing, harm reduction, Trauma-Informed Care and Housing First

2. Links participants to domestic violence and homeless family services resources

3. Provide linkage to shelter, mental health and health services, detox/rehabilitation programs, etc. as well as social service advocacy

4. Facilitate weekly resource table outside of our office

5. Connects/enrolls participants to employment, benefits and documentation

6. Assist Case Management team with Case Management orientation.

7. Create and facilitate wellness groups for participants using SAMHSA's guide to wellness; 8 dimensions of wellness: Social, emotional, spiritual, intellectual, physical, environmental, financial, & occupational

8. Recruit facilitators to provide wellness groups and resource presentations under the 8 dimensions of wellness and based on the needs of the population

9. Conduct Access Center monthly Surveys, review data and in conjunction with the program manager, make changes to the program accordingly to meet the needs of our participants

10. Maintain accurate client records and statistical data (manually and via computerized database system) for all groups and services provided

11. Oversees Narcan distribution to staff and participants and responsible for monthly reporting on Narcan stats.

12. Participate in all case file, staff meetings, supervision, team meetings, training, and retreats

13. Participate in on-going self-evaluation, program evaluation, and development/modification

14. Provide Crisis intervention using Non-violent Crisis Intervention techniques

15. Communicate effectively and promptly with management team, peers, collaborators and clients

16. Other duties as assigned

Qualifications:

1. Bachelor's degree preferred; or a minimum two years' experience working with homeless individuals, preferably those living with mental illness and/or substance addictions, or an equivalent combination of education and experience.

2. One year experience in group facilitation

3. Demonstrated knowledge of issues faced by population served (homelessness, mental illness, substance abuse, medical issues, domestic violence, etc.)

4. Knowledge of social services agencies, community & housing resources

5. Able to work in a high tolerance Harm Reduction model with members who have multiple barriers

6. Ability to self-motivate, learn quickly, work effectively with a wide range of constituencies and be flexible in a fast-paced environment

7. Experience working in a community-based setting and as part of a team

8. Skilled in non-violent crisis intervention

9. Detail oriented with strong time management, organizational, written, verbal, and computer literate (knowledge of Microsoft Word, Outlook, and Excel)

10. Current, valid California Driver's License with an acceptable driving record, reliable vehicle

11. Comfortable, able and willing to drive/transport program participants in agency van

12. Able to provide cleared TB test & pass a criminal background check

Preferred Qualifications:

1. Bilingual preferred

2. Bachelor's degree preferred

Work Environment:

1. Combination of field and office environment

2. Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs.

3. On occasion walk or drive to different local sites

4. Will necessitate working in busy and loud environments

5. Will be exposed to elements like cold, heat, dust, noise and odor

6. May need to bend, stoop, twist and sit throughout the day
